Completed forms can be scanned in and emailed to katy.evans@aht.org.uk, or posted to Katy Evans, Centre for Preventive Medicine, Animal Health Trust, Lanwades Park, Kentford, Newmarket, Suffolk, CB8 7UU. Alternatively, you can request paper copies of these forms to complete by contacting Katy Evans by email or post.

Please could you also send a photo of your hound(s) to Katy Evans by post or email. It would be extremely helpful if the hound’s nose colour was clearly visible in the photo.
