The Grass Sickness Surveillance Scheme is very keen to hear of any grass sickness cases that you have encountered and we have made 2 online case-report forms available for you to report your cases directly to us.
The first of these forms is the 'Premises Questionnaire'. This questionnaire should be completed when the following applies:
- You have not already filled in this questionnaire
- You have had one or more cases of grass sickness on your premises in the past
The second of these forms is the 'Case-report Questionnaire'. This questionnaire should be used when the following applies:
- You have already filled in the 'Premises Questionnaire'
- You have had a new case of grass sickness that you have not reported to us before
- Please note that this form is for one horse only - therefore all new cases need to be filled in on
separate forms
if you have more than one new case to report to us
Although we mainly wish to collect data from the year 2000 onwards, if you know of any cases before this date then we
would still appreciate you reporting them to us.
